How do I complete a checklist?

Checklists can be completed over the phone on 01522 782172 or online at (click on the Get Advice tab). Checklists are available in most public places, such as libraries and GP surgeries. You can request them by calling 01522 782172 or emailing

Is there a charge for the service?

All information and advice provided is FREE. Some organisations, after providing information and advice, may offer a service and in some cases there may be a charge for this.
